Individual Program Plan (IPP): A written plan
is developed by a regional center interdisciplinary team. This
plan states the individual’s goals based on their needs, preferences,
and life choices. It also includes a statement of specific, time-limited
objectives for implementing personal goals and addressing his or her
needs.
Individual Services Plan (ISP): A written plan is developed
by the RES SUCCESS behavior. The ISP states stating a brief individual history,
goals pertaining to eight specified domains, and time-specific objectives
for implementing the goals and addressing his or her needs. The ISP
also contains an individualized plan for each specific goal in an effort
to assist the individual in working towards the objectives.
Quarterly Reports: An evaluation of the individual’s
progress towards each ISP goal is written every three months by the
individual’s instructor.
Daily Progress Notes: Daily tracking of each student
is written by the individual’s instructor. Daily progress notes
consist of the student’s activities, progress, participation,
mood, health and behaviors for the day. Behaviors are individually charted
according to the time they take place and described in “ABC”
format; including what happened before the behavior occurred, the behavior
itself, and the consequence or outcome of the behavior.
Clinicals: Monthly meetings are attended by the clinical
consultant, the behavior analyst, day service program instructors
and supervisors. Clinical meetings provide an opportunity for all the
RES SUCCESS staff to discuss specific concerns, ask questions, share
information, review training and discuss all information relating to
behaviors and clinical issues.
